This is not a contract. Actual terms and conditions are detailed in the contract issued by Green Shield Canada (GSC) upon final application approval. The contract contains important details concerning exclusions, conditions and limitations. Please review them carefully.
Special Benefits Insurance Services is committed to the protection of any personal information collected by it, or in its custody, including any information transferred to a third party to fulfill the purposes for which the information was collected. Personal information includes any information, recorded or not, about an identifiable individual. Examples of personal information that are identifiable include names, age, sex, social insurance numbers, health status, health history, financial information, benefit claims data, etc. It also includes information which can be matched with other information in order to establish an individual’s identity.
Your personal information is collected for the purpose of providing you with health and dental benefits, claims analysis and claim payments. Please read GSC’s Commitment to Privacy.
Coverage is generally effective the first day of the month following the approval of the application.
Medically necessary treatments, services or supplies are eligible for coverage. “Medically necessary” means a treatment, service or supply which is generally accepted by the medical profession as essential, effective and appropriate in the care and treatment of a medical condition, sickness or injury.
You will only be eligible for emergency medical travel benefits if existing or pre-diagnosed conditions are completely stable and you are fit to travel (in the opinion of GSC’s Assistance Medical Team) at the time of departure from your province of residence. GSC reserves the right to review your medical information at the time of claim.
Emergency medical travel benefits do not cover any hospital or medical care for childbirth occurring within eight (8) weeks of the expected delivery date, or deliberate termination of pregnancy.
Reimbursement will not be made for hospitalization due to pregnancy or pregnancy related conditions which commence during the first 10 month period following the covered person’s coverage effective date.
Evidence of health is not required for a newborn child if the application is submitted within 30 days following the date of birth. Upon approval, coverage will become effective on a date to be determined by GSC.
